In the spring of 2018, there were 28 critical incidents that resulted in serious harm to patients according to Manitoba Health’s latest quarterly report.

Three of the incidents resulted in deaths.

All the incidents happened between April 1 and June 30 of 2018 at hospitals across the province, none of which have been named.

The incidents include a missed cancer diagnosis, a patient receiving 10 times the prescribed dose of cardiac medicine, several falls resulting in broken bones, and several other serious incidents.

The Manitoba Institute for Patient Safety says that every incident needs to be taken seriously in the hopes of finding out what went wrong and prevent it from happening again.

The list became public following World Patient Safety Day on September 17th. The day had a theme of “speak up for patient safety.”

Every incident on the list will be reviewed, and then the findings will be reported to the affected families along with an apology.

The Canadian Patient Safety Institute’s patient safety week is the last week of October, and this year’s theme is “conquer the silence.”
